The Application Process In a Few Simple Steps
  1. After you have obtained an application form for each Assistance Program in which you want to enroll, read the instructions on each form very carefully. Generally they require your name, address, contact information. Some forms request income verification such can be your check stub, last years income tax form etc.

  2. Take each application form to your prescribing doctor. He will be requested to provide medical information, include a prescription slip for your drugs. On some occasions he will only have to write a letter on his stationary why he feels you need assistance.

  3. Ensure that your doctor mails the forms to each drug manufacturer to be considered for approval. The drug companies are efficient and quick to approve applications. Some give it over the telephone. Within 2 to 3 weeks the drugs are mailed to either you or your doctors office at no cost to you. Usually a doctor writes a prescription for a 90 days supply of drugs.

    To keep the supply of drugs flowing you should submit a re-application form together with a new prescription slip from your doctor 2-3 weeks prior to exhausting your present supply. Some drug companies automatically include a renewal application in your shipment; with others you might have to call for a renewal form. It's up to you to re-submit your request in a timely manner.

  • Always keep in mind the drug companies rely very heavily on your doctors recommendations so he plays a vital role in your acceptance. If your doctor is unfamiliar with these programs request he call the Director of the Assistance Program directly or call our Resource Center and we can supply that information to him. We want you to have a successful enrollment and save money!

  • Make a copy of all the forms and letters before they are mailed. File your copy in a safe place and use it for your renewal application.

  • If a drug manufacturer declares you ineligible for their program request your doctor prescribe a different or generic medication and then simply re-apply to that new drug company.
